outstanding medical care to each of our patients.

On the left hand side of the screen, you will find pertinent information that you will need to complete and bring with you to your appointment. We provide these forms online for your convience so you will not have to fill them out upon your arrival.

When you arrive, our receptionist will obtain a copy of your driver's license and insurace card(s), along with these forms. Pictured identification is kept in your confidential chart for medical record release purposes. This is for your protection as well as ours. All co-pays or deductibles will be collected at that time. If your insurance requires a referral, you must bring your referral with you prior to being seen.

Please bring x-rays or labs with you and bring a list of all medications you are currently taking.
